In order to process your visit more efficiently and get you into your appointment more quickly, please arrive 20 minutes prior to your scheduled appointment time.

Office & Payment Policies

Thank you for choosing us as your kidney care provider.  We are committed to providing you with quality, accessible, and affordable health care! In an effort to answer your billing and insurance payment questions, we have developed this payment policy page. Please read below, and contact us ask us any questions you may have. 


We participate with most insurance plans, including Medicare and Medicaid.  Please scroll down to view our complete list of accepted insurances. Knowing your insurance benefits and providing us with an up-to-date insurance card (and a referral if required by your insurance carrier) is your responsibility.

Please contact your insurance company if you have any questions regarding coverage. If you are not insured by an insurance plan, we do offer a self-pay option. If you are insured by a plan but do not provide us with an up-to-date insurance card during your visit, you may incur self-pay charges until your insurance can be verified.

Co-Payments & Deductibles

All co-payments and deductibles must be paid at the time of service. This arrangement is part of your contract with your insurance company. Failure on our part to collect co-payments and deductibles from patients can be considered fraud and inducement, both of which are violations of the law. Please help us in upholding the law by paying your copayment at each visit.

Service Coverage

Please discuss and review your insurance coverage with your insurance company to understand what services are covered and non-covered by your insurance.  As medical care providers, we do not have access to your insurance coverage information before you schedule your appointment with us — only you and your insurance carrier have this information.  Per your insurance carrier:  covered services are services that are paid for by your insurance company; non-covered services are services that you are responsible to pay for in full to your medical provider. 

Proof of Insurance

All patients must complete our patient information form before seeing the doctor. We must obtain a copy of your driver’s license and current valid insurance card to provide proof of insurance. If you fail to provide us with the correct insurance information in a timely manner, you may be responsible for the balance of a claim.

Claims Submission

We will submit your claims and assist you in any way we reasonable can to help get your claims paid. Your insurance company may need you to supply certain information directly. It is your responsibility to comply with their request. Please be aware that the balance of your claim is your responsibility whether or not your insurance company pays your claim. Your insurance benefit is a contract between you and your insurance company; we are not a party to that contract.

Coverage Changes

If your insurance changes, please notify us before your next visit so we can make the appropriate changes to help you receive your maximum benefits. 

Payment Challenges

To help you pay-off large sum balances, we offer installment payment options.  

Missed Appointments

Please help us to serve you better by keeping your regularly schedule appointments.  If you do not show-up to your appointment and do not notify us at least 24 hours in advance, you may incur a no-show fee.

Returned Check Fee

If you pay with a check that is returned by the bank due to insufficient funds, you may incur a $35.00 returned check fee. Our practice is committed to providing you with affordable, accessible, quality patient care. Our prices are representative of the usual and customary charges.
